Services Offered at Our Clinic
Services provided in the clinic include routine preventive care for all ages, well-woman exams, management of acute and chronic illness, and military medical readiness. Additionally, the clinic offers Behavioral Health, Case Management, Immunizations, Minor Procedures, Respiratory Therapy, and EFMP services.
Integrated Behavioral Health
Our Integrated Behavioral Health program offers individual, 30-minute appointments to address a range of concerns, from behavioral health conditions like anxiety, depression, and stress, to health conditions with a behavioral component such as tobacco cessation, chronic pain, and insomnia. You'll work with a Licensed Therapist who will provide you with practical, evidence-based skills you can use at home to reduce symptoms and improve your daily functioning. The benefits of this team-based care within your primary care clinic include quicker access to appointments and a solution-focused approach, with no referral required.
Case Management
Our team consists of adult and pediatric nurse case managers. They play a vital role in helping patients identify and manage the healthcare treatments and services needed to improve their health and control their conditions. For more information, please visit

Immunizations
We are dedicated to protecting the health of our community by offering a range of immunizations for patients of all ages, from infants to seniors. Our nurses are available to provide personalized guidance, especially for those who may have fallen behind on their vaccination schedule, helping you navigate and plan any necessary catch-up immunizations to ensure optimal protection.
Minor Procedures
At PCMH, we offer a range of convenient minor procedures directly in our clinic, minimizing the need for outside referrals for many common needs. These services include newborn circumcision, vasectomy, minor skin procedures, battlefield acupuncture, joint injections, and Nexplanon/intrauterine device (IUD) insertions and removals. All minor procedures are scheduled based on a referral received from your Primary Care Manager (PCM), ensuring coordinated and appropriate care.
Respiratory Therapy
We offer respiratory services to address a variety of pulmonary concerns. This includes performing essential diagnostic tests such as Electrocardiograms (EKGs) to assess heart activity, and Pulmonary Function Tests (PFTs), which measure lung capacity and airflow. These in-clinic capabilities allow for convenient and timely evaluation of respiratory health, supporting our commitment to providing integrated and accessible care for our patients.
Exceptional Family Member Program (EFMP) & Overseas Screening
Bayne-Jones Army Community Hospital participates in and enrolls patients into the EFMP program in accordance with AR 608-75. EFMP is a program for family members with special medical or educational needs. Enrollment is completed by the family member's PCM by completing an electronic DD2792 through the E-EFMP portal. All EFMP enrollment, update, renewal, and disenrollment appointments are coordinated through the Military Treatment Facility (MTF)/EFMP Case Coordinator at 726-780-2169. If you need enrollment for a patient who is currently in the hospital, please call 726-780-2169 and ask to speak with the EFMP Medical Director. Questions regarding the EFMP process should be directed to the MTF/EFMP Coordinator at 726-780-2169.
- Family members 18 and older are required to have a DS Login to be able to access E-EFMP to sign their own HIPAA Release and certify their own electronic DD2792.
The
Family Member Travel Screening (FMTS) or overseas screening must be requested through the E-EFMP portal before an FMTS appointment can be scheduled. The FMTS is performed by the EFMP Medical Director or their designee. All family members accompanying the service member to an OCONUS location (including Alaska and Hawaii) must undergo a medical screening to determine if they meet the criteria for EFMP enrollment, update, or disenrollment. FMTS appointments are scheduled through the MTF/EFMP Case Coordinator at
726-780-2169. If a family member is enrolled in EFMP, please contact the MTF/EFMP Case Coordinator to ensure their EFMP status is up to date. Any questions regarding the Family Member Travel Screening or overseas screening process should be directed to the EFMP Coordinator at

Important FMTS Information and Requirements:
- FMTS can be conducted within 12 months of the report date. Getting started early will help ensure a smoother transition. As soon as you are aware of a pending overseas assignment, please contact the MTF/EFMP Case Coordinator.
- Complete an FMTS Contact Sheet and add it to the E-EFMP portal FMTS package.
- TRICARE Prime Members' FMTS Requirements: A current annual physical is required, and medical records from the past 5 years from outside providers may be required, depending on how long family members have been TRICARE Prime.
- TRICARE Select Members' FMTS Requirements: A current annual physical and medical records from the past 5 years are required to be submitted to the BJACH Release of Information (ROI) office at 726-780-2577/2262.
